Referral Form
To request an appointment online, you will need a copy of your referral form. Please take a photo with your phone and upload it along with your booking request. We will endeavour to respond to your booking request within 24 hours.
How to Prepare for your Appointment
Please be aware of the following recommended preparations.
Please drink 2 glasses of water 1 hour prior to your appointment, if you are booked in for one of the following scans:
- Pelvic scan
- Dating scan
- Anatomy/Nuchal scan
- Morphology scan
Please drink 1 glass of water 1 hour prior to your appointment, if you are booked in for:
- Third trimester scan
Please drink 700ml – 1L of water 1 hour prior to your appointment, if you are 18 years or under and booked in for:
- A transabdominal scan
Please be well hydrated, if you are booked in for:
- Non-Invasive Prenatal Testing (NIPT/NIPS) such as Nest and Harmony
- Reproductive Genetic Carrier Screening (RGS)
There is no preparation required for:
- Follicle tracking
